General guidelines prior to
PEME and other Medical Examination
Documents to carry : Photo Identity proof document
- Passport
- CDC
- Company specific requisition
- Admit card (students)
Photograph
3 copies recent colour passport size
Maintain fasting period (nil orally) minimum 12hrs prior to medical checkup
Relevant documents regarding past medical history to be submitted at the Clinic during medical examination
Prescription copy (if presently under any medication) to be submitted during medical examination
Any health related condition / issues must have to be disclosed during medical check-up to the medical officer
